SHIPPER'S LETTER OF INSTRUCTION

1.  The date you are filling out the SLI ( shipper's letter of instruction) . 
2.  Your reference number (how your company refers to this shipment). 
3. The carrier's booking that Alliance Cargo Express will inform you of. 
4. Complete information about the USPPI -who benefits from the export, is the shipper of record and has nominated the agent to file AES. 
5.  USPPI's IRS number (EIN, Tax 10 number). 
6.  Are the USPPI and ultimate consignee related parties? Companies are considered related when either party, the USPPI, and the Ultimate consignee, owns directly or indirectly 10 percent or more of the other party. Source: www.aesdirect.gov/support/userguide.html 
7. Complete the address and contact info for the ultimate consignee. Direct Consumer (D) - A non-government institution, enterprise, or company that will consume or use the exported good as a consumable, for its own internal processes, as an input to the production of another good, or as machinery or equipment that is part of a manufacturing process or a provision of services and will not resell or distribute the good. Government Entity (G) - A government-owned or government-controlled agency, institution, enterprise, or company. Reseller (R) - A non-government reseller, retailer, wholesaler, distributor, distribution center or trading company. Other/Unknown (0) - Other/Unknown is an entity that does not fall under one of the other three ultimate consignee types or whose type is not known at the time of export. 
8. Notify Party - may be the same as the consignee, maybe the consignee's broker/agent. Include a contact name, telephone, and email. 
9. Is the shipment hazardous goods? 
10. Is this a routed shipment? A routed export transaction is a transaction in which the FPPI authorizes a U.S. agent to prepare and file the  EEL While the FPPI will select the U.S. agent, it may give authorization to the USPPI to file the EEL There are separate responsibilities for the U.S. Principal Party in Interest and authorized agent in a routed export transaction. §30.30(e). 
11. Do you want Alliance Cargo Express to insure the shipment?
12. INCOTERM that you are shipping under?
13. Named place that the INCOTERM applies up to. 
14. Description of goods or commodities: What are you shipping? Provide an appropriate commercial description of the goods.
15. D/F: Is your product of Domestic or Foreign origin?
16. The Harmonized Tariff number or the Schedule B number. 
17. The unit of measure is determined by the Schedule B or HTS number entered. To determine the correct unit of measure, consult the US  Census webpage: www.census.gov/foreign-trade/schedules/b/2013/index.htmI 
18. Weight in KG. 
19. Value of that Schedule B/HTS number. 
20. Is an export license required for your shipment? NO: NLR, Yes: provide License Number and expiration date. Exception: Check that box and supply a citation.
21. ECCN: Does your product have an Export Commerce Control Number? YES: provide that number, NO: EAR99. 
22. What is your shipment's US State of origin, and what country are you shipping to?
23.  What are the payment terms for your sale to your customer? (Do we need to be concerned about whether you have a letter of credit or a direct collection letter? 
24. Weights and dims for LCL or Air shipments. Not required for FCL.
25. Do you have any special handling instructions or documentation requirements that you want to make us aware of? 
26. How do you want us to distribute the export documents after sailing? 
27. The signature of whoever within your corporation is authorized to sign. Does have to be an officer of the corporation.
